The Family Practice & Counseling Network (FPCN) provides comprehensive primary care, dental and behavioral health services. The Network consists of three (3) federally qualified health centers (FQHC) in Philadelphia:

  • Abbottsford Falls Family Practice & Counseling (Northwest Philly);
  • Stephen & Sandra Sheller 11th Street Family Health Services of Drexel University (North Philly); and
  • Health Annex (Southwest Philly).

Today the FPCN serves more than 20,000 patients annually and is a NCQA certified Patient-Centered Medical Home and recognized by HRSA as a National Quality Leader for Behavioral Health.

Position Summary

The HRC is responsible for upholding the Health Center's mission by assuring that all clients receive high quality, comprehensive health education within Central High School. This position includes, but is not limited to: providing age and developmentally appropriate health education to students, engage with students to promote increased knowledge and positive health outcomes.

Essential Duties and Functions

  1. Manage the day to day operations of the HRC at Central High School at 25 hours per week, in collaboration with Central High School, The Health Annex and Access Matters
  2. Provide RESPECT sessions to all Central students who qualify for the evidenced-based intervention
  3. Provide comprehensive, high quality, evidence-based sexual health education and counseling in the areas of reproductive health, sexually transmitted diseases, HIV/AIDS, male/female anatomy, sexual orientation, pregnancy prevention, birth control methods, and healthy relationships, etc.
  4. Promote positive attitudes, behavior change and risk reduction techniques, including increasing students' self-efficacy and healthy decision making, reducing STIs, and increasing family planning knowledge and skills, etc. for clients served at Central High School
  5. Refer clients to The Abbottsford Falls Health Center for primary care, dental and/or behavioral health services as appropriate.
  6. Maintain adequate supplies and provide safer sex supplies to eligible students.
  7. Develop and implement a marketing plan for the HRC to promote and increase access to all students who seek out the HRC.
  8. Identify barriers to access the HRC and problem-solve in collaboration with Bartram High School, The Abbottsford Falls, and Access Matters to remove/reduce barriers to access
  9. Complete all required data entry and reports necessary.
  10. Participate in the HRC monthly meetings and in-service trainings, as required.
  11. Develop and participate in health education and outreach activities in collaboration with Central High School and The Abbottsford Falls Health Center.
  12. Provide support for school-based health classes.
  13. Maintain an environment that promotes and supports confidentiality.
  14. Implement and maintain quality improvement activities related to the HRC, in collaboration with Central High School, Access Matters and The Abbottsford Falls.
  15. Maintain current Central High School, Access Matters and The Abbottsford Falls policies and guidelines.
  16. Performs other duties and responsibilities as assigned.

Qualifications

  • 1 - 3 years' experience working with adolescents and reproductive health, preferred
  • Bachelor's Degree in health education, counseling, or related field
  • Excellent organizational skills to manage new project at Central High School
  • Thorough knowledge on reproductive health, family planning methods, sexually transmitted diseases and the ability to relay information to students in an age-appropriate and culturally sensitive manner.
  • Excellent interpersonal, customer service and problem-solving skills.
  • Excellent group leadership and facilitation skills.
  • Proficiency in computer skills and willingness to learn all computer systems.
  • Unconditional ability to maintain HIPAA/confidentiality regulations.
  • Demonstrates cultural competency among persons from various ethnic and social backgrounds.
  • Leadership and high initiative skills that include the ability and desire to champion the HRC program within Central High School.
  • Previous experience working with adolescents is highly preferred.
  • Self-motivated, with ability to work with little direct supervision.
  • Commitment to working with youth in a respectful and empowering way.
  • Responsive to new ideas and situations.
  • Criminal background check and child abuse clearance.
  • Current CPR Certification
  • COVID Vaccinated

Covid-19 Vaccine Mandate: FPCN, a program in the City of Philadelphia, are required to follow the COVID vaccine mandate for healthcare workers per Philadelphia Department of Public Health. FPCN requires Healthcare Workers to be fully vaccinated. Fully vaccinated means it has been two weeks since the final dose of vaccine. Thus, we require vaccination prior to start for all new hires. This currently does not include boosters.
